The most common armodafinil side effects reported in clinical trials are headache (approximately 17% of patients), nausea (approximately 7%), dizziness and insomnia. Armodafinil also carries the same serious dermatologic and hypersensitivity warnings as modafinil.

Serious risks
Serious skin reactions, including Stevens-Johnson syndrome and toxic epidermal necrolysis, have been reported with armodafinil and modafinil. Multi-organ hypersensitivity reactions, sometimes presenting as fever and rash with involvement of other organ systems, have also been reported. Any new rash while taking armodafinil warrants prompt medical attention, since these reactions can progress rapidly and there is no way to reliably distinguish a serious reaction from a benign one at onset.
Psychiatric effects
Anxiety, agitation, irritability and, less commonly, mania, hallucinations or suicidal ideation have been reported with armodafinil, consistent with the psychiatric warnings carried by modafinil. Patients with a history of psychosis or bipolar disorder warrant closer monitoring.
Cardiovascular effects
Armodafinil can produce modest increases in blood pressure and heart rate in some patients. Cardiovascular monitoring is recommended, and caution is advised in patients with a history of left ventricular hypertrophy, mitral valve prolapse, or arrhythmia, mirroring the cardiovascular precautions on the modafinil label.
Comparison with modafinil
Armodafinil and modafinil belong to the same pharmacological class and produce broadly the same spectrum of side effects, both common (headache, nausea, insomnia) and serious (dermatologic and hypersensitivity reactions, psychiatric effects, cardiovascular effects). Reported incidence percentages differ slightly between the two drugs’ labels, which reflects differences between their respective trial populations rather than a fundamentally different safety profile.
